If you are coming from a different PWSA organization you must fill out the PWSA Permission to Play and bring it to the tryout.
These forms must be filled out and signed by an association executive and presented to the association/team for which the player wishes to tryout BEFORE she is allowed on the field. Failure to comply will result in a minimum of 2 months suspension for the offending player and/or head coach.
